Membership Application
Hello Trojans!
The Omicron Epsilon Chapter of Phi Alpha Honor Society is happy to welcome you to the Membership Application. This page will walk you through the process to submit the required documents for consideration. In order to be considered for membership you must meet and show proof of the following requirements:
- Successful completion of 12 credit hours of MSW coursework
- A cumulative GPA of 3.75 or higher
- A letter of recommendation from a faculty member or field instructor
- Please complete and submit this form no later than the DUE DATE announced on this website.

How to Access Your STARS Report
- Log on to MyUSC
- Scroll down to the bottom left of the page and click on OASIS in Student Links
- Once you are on OASIS, click the STARS report link
- At the top of your browser, click file/print
- *Change the printer option to “SAVE AS PDF” and save on your desktop
- Return to the Phi Alpha Application and upload the PDF version of your STARS Report
- Please check to ensure your Student ID Number (#) is visible at the top right corner!
** If STARS report is not uploaded properly we will reach out and request you update it (this is not a copy of your transcript)
Phi Alpha Honor Society By-Laws and Constitution
** By submitting your Membership Application, you have read and agree to the terms as stated in the Phi Alpha Honor Society By-Laws and Constitution.
*** Please note: DO NOT pay membership dues until you have received a formal acceptance email. Per our By-Laws, we cannot issue refunds.*** The application does ask for you to choose a method of payment, we ask that you just select the method that you will be using once approved.
