The Omicron Epsilon Chapter of Phi Alpha Honor Society is honored you are interested in applying for membership. Our membership application is currently closed as we await updates from national.
We encourage you if you haven’t already to submit a New Member Interest Form, so we can effectively communicate with you once applications are open.

This page will walk you through the process to submit the required documents for consideration. In order to be considered for membership you must meet and show proof of the following requirements:
- Successful completion of 12 credit hours of MSW coursework
- A cumulative GPA of 3.75 or higher
- A letter of recommendation from a faculty member or field instructor
- Please complete and submit this form no later than the DUE DATE announced on this website.

How to Access Your STARS Report
- Log on to MyUSC
- Scroll down to the bottom left of the page and click on OASIS in Student Links
- Once you are on OASIS, click the STARS report link
- At the top of your browser, click file/print
- *Change the printer option to “SAVE AS PDF” and save on your desktop
- Return to the Phi Alpha Application and upload the PDF version of your STARS Report
- Please check to ensure your Student ID Number (#) is visible at the top right corner!
** If STARS report is not uploaded properly we will reach out and request you update it (this is not a copy of your transcript)
